Our Healthcare Management services are provided by a Professional Services Director, four Case Managers and two Case Manager Assistants. People receive support with the following:

Medical Oversight including scheduling and transportation to and from all medical appointments.

  • Preventative Screenings
  • Lab work
  • Routine follow up appointments
  • Sick Visits
  • Well Visits
  • Specialist Visits
  • Routine Immunizations

 

Medication Monitoring

  • Oversight of physicians orders and refills
  • Oversight and training for people who Self Administer
  • Administrating Medications (Note: Staff must successfully complete the Medication for the Unlicensed Professional course before being able to administer medications.)

  • Oversight of administrative records

 

Routine Monitoring

  • Blood Pressure Checks
  • Blood Sugar Checks
  • Breathing Treatments
  • Wound Care
  • Oxygen
  • Bi-pap and C-pap
  • Oversight and training for people who self-monitor

 

Hospitalization

  • Admittance
  • Communication with Hospital Staff
  • Discharge Planning

 

Oversight and Implementation of Person Support Plan

  • Monthly Reviews
  • Main contact for individual supported
  • Oversight of main file
