Adult Day Care program offers people:
- A wide variety of activities designed to ensure the person’s health and safety
- Individualized recreational opportunities
- The opportunity to improve fine and gross motor skills
- The opportunity to improve mental and emotional functioning
- Increased socialization
- Encouragement to develop or improve their ability to perform daily living activities, therefore enhancing their independent living skills.
Our Adult Day Care Program is called CHOICES and is funded by Long-Term Services and Support (LTSS) through TennCare in order to provide services to individuals who are elderly (65 years old and above) or have a physical disability and are at least 21 years old.
The program is designed to keep individuals in their homes and communities rather than live in an assisted living facility or a nursing home. The initial application for CHOICES must be made to an individual’s TennCare Managed Care Organization (MCO). Other than the qualifications above, approval from the program is subject to the individual’s income and his/her personal property value limits.
Once an individual is approved for CHOICES, the MCO’s Care Coordinator assigned to that person will assist the individual in locating services that are appropriate to their particular needs. When services are identified, a suitable facility is located and the Care Coordinator files a Plan of Care requesting the desired services to the MCO. The approved request is sent directly to us by the MCO, and we will start providing services to the individual within five days of that approval. Our Adult Day Care program serves people who live in Anderson and parts of surrounding counties. If the parent/caretaker is unable to transport the individual to and from our facility, we will provide transportation to the Center at a minimal cost.
